SCHEDULING

How do I book an appointment?

Booking your appointment with us is easy. You can call us directly at 833-739-3100 between 9am and 6pm Monday to Friday or 10am and 3pm on Saturdays. You can also complete the booking form on our website and a Jack Nathan Rehab representative will reach out to you.

Do I need a doctor’s referral to book your services?

You do not need a doctor’s referral for our services. However, if you are going to use your health insurance to cover the cost of treatment or products, your insurance provider may ask you for a doctor’s referral.

What are your hours of operation?

Our clinic hours vary by location, please visit our contact page for individual clinic hours.  If you wish to contact us to schedule an appointment you can reach us anytime between 9am and 6pm Monday to Friday or 10am and 3pm on Saturdays.

Do you accept walk-ins?

We accept walk-ins and will do our best to see you immediately. If we cannot accommodate an appointment at that time, we will happily schedule you in with our next availability.

To ensure you are able to see a practitioner we do recommend booking in advance. 

APPOINTMENTS

What can I expect on my first appointment?

Please arrive at least 10-15 minutes early for your first appointment to complete the initial intake form. You will be asked a series of questions to gather your medical history in order to help better understand your injury or symptoms.  Assessment and therapeutic treatment will vary; a therapist will suggest a treatment protocol that can effectively treat your injury/pain and or inflammation.

What if I still have questions prior to my appointment date?

There will be time prior to the start of your treatment for a member of our qualified allied health team to answer any questions you may have.  We also invite you to contact us directly at 833-739-3100 with any immediate concerns that you would like us to address.

What if I am unable to keep my appointment?

If you are unable to keep your appointment please notify us immediately at 833-739-3100.  Appointments that are cancelled less than 24 hours in advance as well as no-shows could be subject to cancellation fees up to the full amount due.

Jack Nathan Rehab proudly offers a full range of highly personalized treatments and products to address various injuries, aches, pains and or inflammation. Who performs my treatment?

Depending on the type of service you will be undergoing, your treatment may be performed by any of the following highly qualified team members:

  • Physiotherapist
  • Chiropractor
  • Registered Massage Therapist
  • Kinesiologist

All therapists at our clinics are registered by their respective colleges and many have extensive years of experience in their field.

How many treatments will I need?

During the first assessment visit, your practitioner will assess your condition, devise a treatment plan and communicate recommended treatments with you.

PAYMENTS

How much do your rehab services cost?

Our prices vary depending on the treatment being provided and the length of the appointment. It is our goal to provide competitively priced treatment options to suit everyone’s budget so patients can get the help they need.

What forms of payment does Jack Nathan rehab accept?

We currently accept cash, debit, all major credit cards and extended health care benefits (when applicable). Kindly note that none of the treatments we currently offer are covered by OHIP.

Do you accept health insurance plans?

Our treatments are covered by extended health care benefits and in most cases we can direct bill your insurance provider.

COVID-19

What are your COVID-19 protocols?
  • Customer facing staff are required to be vaccinated against COVID-19.
  • All staff are required to wear face coverings and/or shields.
  • Clients are required to wear a mask or face covering while in the clinic. Please bring one with you to keep everyone safe. Masks can be removed if required to administer a treatment.
  • We deep-clean and disinfect as directed by public health between each client.
  • Sanitation wipes and hand sanitizer stations are available for clients in our clinics.
Are your staff vaccinated against COVID-19?

Yes, all Jack Nathan Rehab customer facing staff are required to be vaccinated against COVID-19 and show proof of vaccination status.
