A few weeks ago, I had A 72-year-old female (Retired) patient visit with a chief complaint of left knee pain because of strain on her Montreal trip, difficulty in walking and standing longer and all her activities of daily living were hampered.

 

PHYSIOTHERAPY ASSESSMENT

I started with the objective assessment, and she mentioned that she had been experiencing this pain and mobility restriction in the last 2 months, it was progressive. Her family doctor referred her for Physiotherapy after reviewing her USG which mentions degenerative changes and fluid in the left knee joint. She graded her pain on NPRS as 5/10, with on/off episodes of sharp pain. Her aggravating factors were walking and standing for more than 10 mins, sitting to stand and she felt relieved with painkillers and heating pads. Due to all these issues her social life was majorly impacted, and she felt she would not be able to feel and walk like before. 

 

The clinical signs from my detailed assessment were mild swelling, joint line tenderness, reduced patellar mobility, knee range of motion was restricted and reduced muscle strength, and positive orthopedic special test. She had a limping gait pattern and frequent episodes of getting unbalanced.

 

REHABILITATION

After discussion and education about conditions, the goals were:

-To reduce pain and swelling

-To improve muscle flexibility

-To improve joint mobility and muscle strength

-Return to an active lifestyle

 

I incorporated my techniques to help in reducing pain like soft tissue release, Joint mobilization, stretching exercises and strengthening exercises (Level 1) and gradual progression done along with Electrotherapy. I tried knee tapping to protect the knee during her acute phase. Lastly, I progressed her session with some Proprioceptive training by using a balance board.

 

RESULTS

After 7 sessions, she was feeling much better. She felt a reduction in pain and improved mobility. She was extremely glad after proprioceptive training that now she can walk on uneven surfaces without straining her knees. She resumed her volunteering at the hospital and enjoying her daily exercise workout.
